The regular diminutive would be "tibiola", because the grammatical stem ending of "tibia" is "-ia-", but "tibiella" is the alternative diminutive formed with an exchange of the diminutive base "-ul-" (in this case "-ol-") with the diminutive base "-ell-". In any case, the diminutive takes the grammatical information from the primitive, thus the diminutive of "tibia" is feminine, irrespective of its formation as "tibiola" or "tibiella". It is thus a noun in apposition and does not change with the gender of the genus.
